Clinical Pathway: RP’s Clinical Value Team presents best practices for Virtual CT Colonography

Radiology Partners recently created a Virtual CT Colonography Clinical Pathway.

Drawing on their extensive expertise, RP’s national subspecialty division (NSD) advisory board for body imaging collected practical guidance for virtual CT colonography (VC) programs including examples of workflows, order forms, patient prep and more. This reference material is geared to serve practicing radiologists and operational leaders when establishing and advancing VC programs at their institutions, which plays a crucial role in early detection and prevention of colorectal cancer.

Effective January 1, 2025, the Centers for Medicare & Medicaid Services (CMS) includes coverage for virtual CT colonography for colorectal cancer screening. Colorectal cancer is the the third most common cancer diagnosed in adults in the United States. This CMS decision marks a significant advancement, providing Medicare patients with access to a minimally invasive screening tool that can detect pre-cancerous polyps without anesthesia.

Based on Nguyen et al.’s “Multidisciplinary consensus on the classification of prenatal and postnatal urinary tract dilation (UTD classification system)” and the extensive expertise of RP’s national subspecialty division (NSD) for pediatric radiology, RP’s Clinical Value Team is pleased to offer practical guidance for neonatal UTD and clear follow-up instructions for patients and referring clinicians.

Dr. Rich Heller leads RP’s NSD for pediatric radiology. In partnership with an advisory board of practicing pediatric radiologists, he spearheads the development and implementation of programs with a mission to enhance clinical value and quality in pediatric radiology across RP. The NSD focuses on refining best practice recommendations, advancing image quality and patient services and aligning with the most current industry standards. Radiology Partners (RP) recently unveiled a Neuroradiology and Stroke Clinical Pathway.

Drawing from current guidelines and the extensive expertise of RP’s national subspecialty division for neuroradiology, RP’s Clinical Value Team is pleased to offer practical guidance for acute stroke care. This clinical resource includes:

  • Operational insights for the entire stroke team
  • A triage checklist for nurses
  • Imaging checklists for technologists
  • Tips for interpreting radiologists

RP’s national subspecialty division for neuroradiology is led by Dr. Vivek Bansal and a five-radiologist advisory board. This group of practicing neuroradiologists are subject matter experts who support the efforts of the Clinical Value Team. They identify best-in-class clinical programs, tools and efficiencies to improve clinical value and quality of neuroradiology throughout RP’s network. The team leverages the scale and expertise from across RP to share education, tools and processes so that all practices can deliver high-quality subspecialty care to patients in their communities.

Rad to Rad Learning cases are shared across RP’s network of 3,900 radiologists and externally across social media channels to ensure accessibility across the entire specialty to elevate care delivery.
  • Best Practices for Breast Biopsy Clips: RP’s national subspecialty division for breast radiology created best practice guidance for breast biopsy clips. This resource includes guidelines for breast biopsy clip documentation, a breast clip education handout for patients and clip shape resources.
  • The Clinical Evaluation of AI Models: Introducing RP’s Five-Step Validation Process: RP’s AI experts created a five-step validation process for evaluating AI models. This comprehensive framework ensures the AI models RP deploys are deemed reliable and trustworthy by radiologists, enhance radiologists’ workflows and provide advanced insights to radiologists to further clinical quality and patient care. This clinical resource contains a summary outlining the five steps and corresponding video presentation describing these steps in further detail for fellow radiologists.
  • Best Practices for Neonatal Urinary Tract Dilation: Based on published data and consensus papers, RP’s national subspecialty division for pediatric radiology developed practical guidance for neonatal urinary tract dilation (UTD) with clear follow-up instructions for patients and referring clinicians, as well as a summary one-pager and sample radiology report template. The UTD classification system creates a standardized approach to evaluating the pediatric kidney and communicating the findings to the referring care team, helping improve patient care.
  • Practical Guidelines for Acute Stroke Care: Drawing from existing guidelines and the extensive expertise of the national subspecialty division for neuroradiology, RP released a Neuroradiology and Stroke Clinical Pathway, which offers practical guidance for acute stroke care. This Clinical Pathway offers operational insights for the entire stroke team, a triage checklist for nurses, imaging checklists for technologists and tips for interpreting radiologists.
  • Power of Collaboration: Revolutionizing Radiology Reports for Referring Physicians: To better serve referring physicians, RP redefined the delivery of radiology reports by creating and implementing report formats that better reflect the preferences of referring physicians. The practice utilized its network of radiologists to gather feedback, then deployed a set of three sample reports to nearly 600 referring clinicians across the country and asked them to rate the reports by: 1) Appearance, 2) Formatting, and 3) Satisfaction of the detail level included. From the feedback garnered, RP was able to pinpoint the report of preference for referring physicians and offers it as a free resource on the website today.

2024 marked the beginning for RP’s public clinical resources offerings. The practice will continue advancing its mission through the development and deployment of educational resources at scale, establishing best practices and building value across the specialty.

RP’s Clinical Value Team: Advancing value for RP radiologists and the entire specialty

Since 2020, the Radiology Partners (RP) Clinical Value Team has worked to advance its mission to elevate patient care and enhance value through innovation, collaboration and education.

Recently, we spoke with Dr. Krishna Nallamshetty, Chief Medical Officer at RP, about the team’s continuing efforts to enhance offerings for RP radiologists and extend valuable resources across the entire specialty.

Accelerating Best Practice Recommendations

Approximately 3,000 radiologists across RP-affiliated practices utilize RP’s Best Practice Recommendation (BPR) program. Managed by the Clinical Value Team, RP practices implementing BPRs are enabled to elevate care by reducing variability and enhancing quality by following evidence-based guidelines. “Recently, we introduced the renal lesion BPR, and it has been adopted by 44 of our practices,” Dr. Nallamshetty said. “Our team also collaborated with our revenue cycle management team to properly align the new BPR with MIPS Measure #405, which is aimed to reduce unnecessary imaging and clinical workup.” Throughout 2023, the number of radiologists participating in our BPR program grew by 67%, and the number of impacted patient exams grew by 170%. Even with this significant expansion in one year, BPR adherence still grew by 4.5%. Additionally, the adherence to appropriate follow-up for incidental thyroid nodules (ITN) has exceeded 75% for five months which would translate to a meaningful reduction in unnecessary follow-up imaging and biopsy procedures. “We believe this is one of the best adherence rates in the industry,” Dr. Nallamshetty said.

More than 2,900 RP radiologists utilize recoMD®, RP’s proprietary AI tool developed by radiologists for radiologists that increases the standardization of patient care and enhances workflow. In 2023, the Clinical Value Team partnered with RP’s Digital Innovation Services and Product Engineering team to increase overall functionality and enhance the user experience for radiologists.

Sharing clinical resources

The Clinical Value Team published its first free public clinical resource, sharing optimized radiology report templates. These report templates were based on a survey of nearly 600 referring physicians that gathered feedback and insights on radiologist reports. “The goal of creating these optimized report templates is to ensure they benefit patients and referring clinicians,” Dr. Nallamshetty said. In the first month, more than 100 individuals from across the United States downloaded our templates for use at their practices.

The team also published a second public clinical resource, this time focused on acute stroke care. This was developed by the neuroradiology national subspecialty division (NSD) advisory board, made up of subject matter experts from all over the country. “The group of practicing neuroradiologists drew from current guidelines and leveraged their own extensive expertise to produce a stroke clinical pathway,” Dr. Nallamshetty said. “This resource provides practical operational and clinical guidance for hospitals, referring physicians, radiologists and other key care team members.” Within two weeks, it garnered close to 100 downloads and earned recognition in various trade publications.

Continuing education

The Radiology Partners Research Institute (RPRI) published a manuscript in Radiology Research and Practice, focused on the pandemic’s effect on mammographic imaging volumes. Four more manuscripts are in review by various journals, and an additional 13 are being finalized for submission in 2024. RPRI also initiated two pilot clinical trials for an absorbable IVC filter and an embolic device for IR procedures within RP practices. Finally, RPRI launched the 11th RP residency program at the University of Texas at Dell Medical School (ARA), raising RP’s resident total to more than 200, with more programs starting in 2025. This commitment speaks to RP’s strong commitment to graduate medical education, as well as the quality of training and experience residents gain. In fact, 43% of these trainees choose to stay within an RP practice to begin their careers.

RP’s National Patient Safety Committee launched a project in 2023 aiming to improve patient care through expansion of peer learning programs. “Since its inception in 2019, the committee has expanded to broaden the team’s subspecialty expertise, and now, they are creating educational materials based on peer learning workflows for all RP radiologists to access,” Dr. Nallamshetty explained.

Advancing clinical value

In 2023, the Clinical Value Team launched a new clinical reference tool available to RP radiologists, where they can easily access commonly used references, grouped by subspecialty, which reduces browser searches and saves them time and energy. New resources are added regularly, and the team worked to integrate these references into the recoMD® platform.

Finally, RP’s NSDs and Clinical Value Team produce regular subspecialty-focused communications for 3000+ RP clinicians. These educational materials include local practice stories, resources and tools, clinical news, volunteer opportunities, interesting cases, peer learning and relevant recent publications.
